Community Health Worker Training Program

The Community Health Worker training model is designed to provide new and current community health worker staff with the necessary theoretical and practical knowledge to address a number of health needs in the community.  Through ten training modules, participants will gain necessary core competencies and knowledge of the communities and populations they support.

This training is designed to offer a broad range of basic practical education to community health workers, care coordinators, service navigators and any health support staff who are dedicated to improving the health outcomes of their clientele.

ABOUT THE CURRICULUM

Community Health Overview
Provides an overview of the healthcare system and population health management for improving community wellness.

Community Health Worker Scope of Practice
Reviews the role of the Community Health Worker in the healthcare and social services world.

Health Coaching
Support patients in setting goals, identifying strengths and motivations, and encouraging the development of healthy behaviors.

Motivational Interviewing
A person-centered approach to counseling patients that conceptualizes motivation as a dynamic and changeable state within the person.

Social Determinants of Health and Cultural Competency
Patient-centered care is communication-based and is the front line to addressing issues contributing to health disparities.

Health Literacy
Examines the degree to which individuals have the capacity to obtain, process and understand basic health information.

Behavioral Health and Substance Abuse Disorders
Provides a foundation for understanding people with mental health and substance use disorders in a community setting.

Assessment and Referral
Reviews the identification of tools for monitoring and measuring performance, including performance indicators.

Obstacles to Primary Care
Reviews the challenges persons with health disparities face accessing primary care and strategies to overcome them.  

Workplace Competencies
Provides aspiring or incumbent Community Health Workers with the skills necessary to be effective in their role.
